Submit entry information
Upload your entry information through the form below: completed entry forms, photos, videos, semen report/test, embryo certificate or any other information you need to submit.
- Enter the requested information, then click on the "Choose File" button to browse your device for the file.
- Please identify the name of the entry in the file names for photos and videos.
- You may upload one or two separate files in each submission. Note the size limits on the files, especially for videos.
- Once you hit SUBMIT, be patient if you are uploading large files. You will get a message that your file has been submitted when it is complete.
